Elementary School Counselor Perception data is collected once a year, at the end of every school year, to determine what 2nd and 5th Grade students at each Elementary School Site in Sunnyside Unified School District think about the services provided by the School Counselor at their school site.

     School Counselors randomly administer perceptions to 25 students in 2nd Grade and 25 students in 5th Grade using the appropriate Student Perception Survey Forms.  

     Student responses are totaled up using the Perceptions Survey Results Excel Scoring Sheet: 

  • School Counselors can enter a student's response by placing a under the Y (“yes”), N (“no”), or U (“unsure”) cell boxes for each of the 25 Student Survey #’s (ex. S-1) for every survey question.
  •           As each 1 is entered under the responses for each of the 25 Student Survey #'s, the form will automatically total up the number of Y’s, N’s, and U’s for each Question #, and will also give a percentage out of 25 students for that total. 

       Number and percentage totals are then entered into the Perceptions Reporting Form, which is submitted to Central Administration (NJ Utter) upon completion.
