  • General Health information

     

    The primary purpose of the Health Office is to assist students with a preventative health program. The office is staffed by a Registered Nurse, available for first aid, assessment of sudden illness and monitoring students with chronic health problems. The Nurse also provides hearing and vision screening, administration of medications and maintenance and updating of immunization records in compliance with Arizona State Law. A student, parent, guardian, teacher or any school staff may request the Nurse’s assistance with a student health matter.

    According to Sunnyside School District #12 policy, medications may be administered by the School Nurse during school hours when the following criteria are met: (1) a written prescription from the child’s Health Care Provider (pharmacy label will satisfy this criteria-Student’s name, Prescriber’s name, Name of medication, Dose and time to be given), (2) medication must be received in the original container. (3) Parents must sign a medication administration consent form. (4) Parents not students must transport medications to and from school.

    In specific instances, with the appropriate documentation, it may be permissible for your child to carry a medication and self-administer his/her medication. If you believe this is the case for your child, please contact the School Nurse for further information.

    When an accident occurs on campus or the student becomes ill, they should report to the Health Office with permission from their teacher. For the student’s protection and safety, all students must report to the Nurse before leaving campus for an illness or injury. The Nurse is responsible for contacting parents/guardians.

    Students should not be sent to school if they have a fever (100 degrees Fahrenheit or greater) a rash of undetermined cause, vomiting or diarrhea. They should also not attend school if they have conjunctivitis (pink eye), chickenpox, impetigo, ring worm, head lice or any other communicable disease unless they have been treated medically or are symptom free. Please request a medical release from your Health Care Provider to assist with a flawless return to class. After having a fever, students need to be kept at home for 24 hours fever-free. A student who vomits, has diarrhea or a fever of 100 degrees or greater at school will be sent home.

    Please make sure all parent contact information-phone numbers and emergency contacts are kept current with the school office. This information is critical for good communication between parents and the school staff.
